If the fracture is not serious and causes no displacement, it can be treated non-surgically by immobilization with cast. When the fracture is displaced or even dislocated, operative reduction and fixation will be considered. Internal implants such as pins, screws, wires and plates are used for internal fixation to hold the bone in place while it heals.
In complicated situations such as severe open fractures, comminuted fractures or when there is soft tissue defect, extra procedures such as bone grafting, external fixation frame or microvascular reconstruction may be necessary in stages.
It is an operation used to fix serious fractures that cannot be treated with a cast or splint. “Open reduction” means to make an incision to re-align the bone. “Internal fixation” means the bones are held together with implants such as metal screws, plates, rods or pins. In most cases, the implants are not removed after the bone heals
The ankle joint is the largest, heavily loaded articulation in the foot. Most ankle fractures are as a result of indirect injury, from either an internal or external force of twist, turning or rotation. When serious it can lead to dislocation or even open fractures.
Most hip fractures result from low-energy falls in elderly patients who have weakened or osteoporotic bone. In some cases, the bone may be so weak that the fracture occurs spontaneously while the person is walking or standing. Fractures in the femoral neck from repeated impact are often seen in long distance runners. Non-operative management is only appropriate in a small group of elderly patients who are non-ambulators prior to fracture and the fracture caused minimal discomfort, or are medically unfit for surgery. Most patients are treated by operative management, which allows early mobilization and prevents prolonged bed rest causing other morbidities. Pins, screws, rods or plates are used for internal fixation to hold the bone in place while it heals. It tends to be used for either
Hip joint is often described as a “ball-in-socket” joint. The “ball” is the femoral head, which is the rounded upper end of the femur (thigh bone). The “socket” is the acetabulum of the hip. The acetabulum surrounds the femoral head, allowing it to move as the leg changes positions. Hemi means half, and arthroplasty means joint replacement. Hemiarthroplasty is performed to treat femoral neck fracture while the acetabulum is intact. The operation involves removing and replacing the broken femoral head with a prosthetic femoral head which articulates with the native acetabulum.
